All coaches for Fall 2008 will need to fill out the Risk Managements/Volunteer Disclosure Forms again starting on June 1, 2008. 

You can fill out the forms in one of the following ways:

1.  Filling out the  ISA Online Risk Management Form.  This goes directly to ISA. 

2.  Printing the Risk Management/Volunteer Disclosure Form  AND the Code of Conduct Form, filling both out completely and either mailing or faxing both forms to  VSA @ PO Box 854 Waukee, IA 50263.

COACHES-- The Fall 2008 Coaches Meeting will be Thursday, August 14 2008 at 6:30  pm at WAUKEE ELEMENTARY in Waukee.    Please note location change!   Plan to attend!  

Player Cards

Why do we need player cards?
Player Cards are used as proof that the players and coaches are registered with the Vision Soccer Academy of Waukee and the Iowa Soccer Association (ISA) for the soccer season (e.g., Fall 2007 through Spring 2008, is one season to ISA). The referees are to check the player cards against the official roster before every game to make sure the appropriate players are playing at that game. They are especially important when traveling to a different club’s fields to show that the player is registered and insured with our club. Since our U6’s do not travel, we only give the player cards out to our U8 and older teams.

What are we supposed to do with the player cards?
The player cards have two pieces. Both pieces have one side with the player or coach’s team information on it. One of the pieces is the “Member Pass”; on the back of it is room for a photo, the member signature and the ISA Registrar’s signature. The other piece is the “Membership Card” which has the Code of Conduct on the back of it. A photo of each player and their signature is required on their own card and each card is to be laminated.

New player cards are given at the start of every fall season and used through the next spring. DO NOT THROW/GIVE AWAY (to any player or coach) ANY OF THE PLAYER CARDS AFTER THE FALL SEASON!!!! Make sure to turn in all player cards to the club after the fall season, we will redistribute them for the spring season.
